Sixteen years ago, OnPointe (under the Heritage Management brand) began to create and implement our first cross-continuum network. This was well before most had heard of Accountable Care Organizations, bundled payments, or gain sharing. The network of services was born of the idea that there had to be a more person-centered way to provide care services to seniors. Simply put, a suite of services that allows the service package to fit the patient’s needs, instead of the other way around.
